Ready to get back on the field, court, or track? 🏃‍♂️🏀⚾️ Jay County Jr-Sr High School, in partnership with IU Health, is hosting FREE Athletic Sports Physicals for the upcoming 2026–2027 school year! _______________________________________________________________________________________________________________________________ 📅 When & Where Date: Wednesday, May 13th Time: Starts at 6:00 PM (Must be in line by 7:30 PM) Location: Jay County Jr-Sr High School – Main Gym Entrance: Please enter through Door 9 📋 Important Requirements Before arriving, Final Forms must be completed. 1. Log in/Create Account: Select the "2026-2027" school year. 2. Complete Forms: Both parent and athlete sections must be finished. 3. Check Status: Ensure all forms are marked GREEN (Yellow or Red means they are incomplete!). 4. Print: Bring a printed copy of the IHSAA PPE Physical Form for the physician. ⚠️ Note for Parents Per IHSAA rules, any student participating in sports for the 2026-27 school year must have a physical dated April 1, 2026, or later.
